About the Role

We are seeking experienced Medical Chart Reviewers with professional coding or clinical expertise to support a healthcare AI training and evaluation project.

Your work will help identify coding discrepancies, documentation gaps, and clinical inconsistencies while improving the accuracy and reliability of AI-generated healthcare outputs.

This opportunity is suited to certified medical coders, Registered Nurses, and clinical professionals with experience in medical chart review, coding audits, or Clinical Documentation Improvement (CDI).

Previous AI experience is not required. Your clinical or coding expertise is what matters.

Key Responsibilities

  • Review and audit medical charts, including admission notes, progress notes, operative reports, consultations, laboratory and imaging results, and discharge summaries.

  • Assess documentation for accuracy, completeness, and consistency with the patient's clinical record.

  • Validate diagnosis and procedure codes, code sequencing, and Present on Admission (POA) indicators.

  • Verify MS-DRG assignments against clinical documentation and applicable coding standards.

  • Identify under-coding, over-coding, missed Complications or Comorbidities (CCs), and Major Complications or Comorbidities (MCCs).

  • Apply ICD-10-CM/PCS Official Guidelines, AHA Coding Clinic guidance, and clinical validation criteria.

  • Identify documentation gaps, conflicting information, and opportunities for provider queries.

  • Review complex cases and communicate findings to project stakeholders.

  • Provide clear, actionable feedback to improve review guidelines, workflows, and AI model performance.

  • Maintain consistent standards of accuracy and completeness across all chart reviews.

Required Qualifications

Candidates should have one of the following professional backgrounds:

  • Active CCS (AHIMA), CIC (AAPC), or CPC (AAPC) certification; or

  • Registered Nurse (RN) or another relevant clinical credential with medical chart review or CDI experience.

Additional qualifications include:

  • Strong knowledge of ICD-10-CM, ICD-10-PCS, and MS-DRG methodology.

  • Hands-on experience in medical chart review, coding audits, or Clinical Documentation Improvement.

  • Ability to interpret complex clinical documentation and identify coding or documentation discrepancies.

  • Understanding of POA indicators, CCs, MCCs, and clinical validation principles.

  • Familiarity with current coding standards and AHA Coding Clinic guidance.

  • Strong analytical, problem-solving, and attention-to-detail skills.

  • Excellent written and verbal communication abilities.

Preferred Experience

  • Medical chart review across acute care facilities, teaching hospitals, or specialty units.

  • Exposure to complex surgical, trauma, oncology, cardiology, or transplant cases.

  • Experience reviewing different types of clinical documentation and resolving challenging coding or clinical validation issues.

  • Access to current ICD-10-CM and ICD-10-PCS code sets, Official Coding Guidelines, and MS-DRG grouper tools.
